The first step towards a beautiful, healthy smile is to schedule an appointment. Please contact our office by phone or complete an online appointment request.

Patient Forms

If you are a new patient to our office, please visit our Patient Forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival. Payment Options

As a convenience our office will process your insurance claims. We accept payment directly from your insurance company. You are expected to pay all fees that are not paid by your insurance carrier. At the time of treatment you are expected to pay an estimate of the non insured portion of the fees.

We accept cash and check payments. As well as credit card payments from Visa, Master Card and Discover.

As a service we also provide financing through Care Credit.

Dental Insurance

We accept most forms of dental insurance that allow you to choose your own dentist. This includes Delta Dental. If you have any questions about your coverage you may contact our office or your insurance provider.
